Parental Consent Form (Annex-1)
This form is an official document that must be completed and signed by the parent or legal guardian to allow the student to participate in the competition.
The consent form must be submitted fully completed and signed during the application process.
Applications submitted without a valid parental consent form
will not be evaluated.
